Annual financial reporting forms will be prepared by the treasurer of the 4-H Club and forwarded to the Regional Program Coordinator at the close of the statewide 4-H YDP year (July 1-June 30) and submitted to the UCCE county office no later than September 1.
All 4-H Club with bank accounts are required to complete annual Financial Reporting. Financial Reporting includes completing following year-end financial reporting documents: Annual Inventory Report (Form 6.2), Annual Financial Report (Form 6.3); Year End Peer Review Report (Form 8.5); Year End Peer Review Checklist (Form 8.6); Outreach Methods and Documentation and a proposed budget for the upcoming program year. The information from these forms must be inputted into the online 4-H Annual Financial Reporting System and the completed forms must be submitted to the 4-H Office by September 1st.
